Request Information
The following three documents regarding safety may be submitted electronically. Please take a few minutes to complete them and return before your arrival at Cottey. Questions about the forms may be emailed to Mike Shuster at mshuster@cottey.edu.
- Emergency Contact Form: Student Contact Information. Please follow the link, complete, and hit the submit button.
- The Missing Person Contact Form: Missing Person Form. Please follow the link, save to your desktop, complete, and email as an attachment to Mike Shuster at mshuster@cottey.edu.
- The Vehicle Registration Form: Car Registration. Please follow the link, save to your desktop, complete, and email as an attachment to Mike Shuster at mshuster@cottey.edu.
The Missing Person Contact Form and the Vehicle Registration Form may be electronically signed by using Adobe Acrobat Signature. Adobe Acrobat signing assistance is available or download a free version of Adobe Acrobat.
We take safety seriously at Cottey and want you to do the same. To help maintain a level of safety that promotes a healthy learning environment requires the assistance of the entire Cottey community. This first starts with all of us working together to promote dignity and respect in all interactions with each other. We further ask that if you observe anything that does not look right let us know. Security personnel are available 24 hours a day at, 417-448-1448, or at extension 2222 from any campus phone.
The Emergency Contact Form is information used to contact students and/or their families in case of a campus or personal emergency, and to conduct College business. This information will be made accessible only to employees of the College.
The Missing Person Contact Form is information we are required to collect if you are a missing person. If you are under 18 years old and not emancipated we must also, within 24 hours, notify a custodial parent or guardian. Please use this form to designate the individuals you wish to have contacted in the event you are a missing person.
The Vehicle Registration Form is completed if you plan on bringing a vehicle with you to obtain a free parking pass. Vehicles parked on Cottey College property, without a pass, are subject to being towed at the owner’s expense.
